Disease Management Program

Your health is important to us. That's why we offer a disease management program for asthma, diabetes, heart failure, coronary artery disease and chronic obstructive pulmonary disease - as well as a number of other conditions, from osteoarthritis to lower back pain.

A Collaborative Effort

The disease management program is a collaborative effort between you, your physician and our disease case managers. You and your physician remain in charge of your treatment plan. The program supports your treatment plan by:

Providing you with helpful information about your condition
Encouraging you to monitor your condition and stay on your treatment plan
Updating your physician on your condition between office visits

We help you learn about your condition, make informed health care decisions, stay healthy and avoid unnecessary medical costs.

How the Program Works

We identify potential candidates for the program using medical and prescription drug claims information and then contact candidates and ask them to participate. But the program is completely voluntary. You choose whether or not to participate. Once enrolled, you'll receive:

A welcome kit and a call from the program to discuss medical history, lifestyle behaviors and current disease state
Follow-up calls to monitor your health and support your physician's treatment plan
Toll-free access to a program nurse
Educational materials
